The Mammovan is returning to Hawthorne! It will be here at Mt Grant General Hospital on November 6 from 7:40am to 11:40am.
For an appointment, please call 877-581-6266 and press option 1 when prompted. Hours to call are Monday - Friday 7am to 6pm.
Prior to your appointment, please fill out the required patient registration forms, which can be found online at: nvhealthcenters.org/mammography/
When you arrive for your appointment, you check in directly with the Mammovan, not with Mt. Grant's front office. You will need to bring a valid government photo ID, an insurance card (if applicable), your FILLED OUT patient registration forms, and the name and phone number of your primary care physician.
